Step 2: You must use the payment method below – this is a separate step from the order form on the left. Use contact information SPECIFIC TO SHIPPING on this form. 

*Note: You will be sent an email proof prior to order completion. After proof is APPROVED, your order will arrive in less than 2 weeks to the shipping address you specify along with the purchase information.
